Telehealth Informed Consent

    Introduction

    This Telehealth Informed Consent describes The Pep Report's role when you book a telehealth visit through our platform. It supplements, but does not replace, the clinical informed consent your treating provider will separately obtain and document at the time of your visit, as required by law.

    1. Nature of Telehealth

    Telehealth visits are conducted remotely by an independently licensed healthcare provider, currently through our partner FUSE Health Inc. and its network of licensed clinicians ("Provider").

    2. Independent Provider Relationship

    Your Provider is independently licensed and makes their own clinical decisions. The Pep Report and DigitalTreehouse LLC do not practice medicine and do not control your Provider's judgment. We reserve the right to change our telehealth or pharmacy partners at any time; your care and prescription history will be handled in accordance with applicable law if this happens.

    3. Risks and Limitations

    Telehealth carries some risk of technical interruption or unauthorized access to electronic communications. Your Provider may determine your condition requires in-person care and will refer you accordingly. You have the right to request in-person care instead of telehealth at any time.

    4. Prescriptions and Controlled Substances

    Prescribing decisions are made solely by your Provider. Some medications may be compounded and are not FDA-approved (see our Terms of Service for details). Federal rules currently allow certain controlled substances to be prescribed via telehealth without a prior in-person visit; this flexibility is temporary (currently authorized through December 31, 2026) and may change, which could affect your ability to continue a treatment without an in-person visit.

    5. Your Responsibilities

    You agree to provide accurate, complete health information. Inaccurate information may affect your Provider's ability to safely evaluate and treat you.

    6. Emergency Care

    Telehealth is not for emergencies. Call 911 or go to the nearest emergency room.

    7. Voluntary Consent

    Participation is voluntary. You may withdraw at any time by contacting your Provider or us, though this may affect an active treatment plan.
